Proof of resident

I need a proof of resident.
that is a offical letter shows the address I am living.

Does anyone know where can I ask for this Proof of resident.

I am living in North York.
Please give me an email, if you know it.

Do you need it for your health card application?

If so, bank statements, telephone bills, hydro bills, driver's license or anything showing your address will do.
