我觉得加拿大的医疗体制就好像美国的三大汽车厂。只是汽车厂面临着全球的竞争倒下了，而加拿大的医疗是垄断。现在病人排长队等待手术，有的甚至半年一年或者更长时间的等待。很多在等待中死亡的事情发生。有钱的到美国自己找医生解决，没钱的只好在加拿大慢慢等待。

三大汽车厂的CEO即使在经济很不好的情况下仍然不能接受降薪，不能接受不坐私人飞机。他们说（no negotiable!）

加拿大医生说只能工作五天，每天8小时，（no negotiable!）。即便看着病人排长队等死。

所以当我看到那么多研究人员在做着各种数学模型，什么排队轮，马可夫链，MIP,MLIP,等很复杂的数学模型来研究怎么提高医疗体制效率的时候，在学术期刊发表了很多研究成果，但是只有极少数的研究被采用实施。

诚然，理论研究和实际操作有gap。但是同样的模型研究方法用在生产优化，供应链优化的实施转化率要高很多。

道理很简单，因为后者实施的主体是机器操作工人，仓库工人，long haul driver，这些没有特权的人，为了保住职位，会配合。医生护士形成了小的利益保护体，就好像三大汽车厂的工会。工会最后把整个北美汽车行业拖垮。医生护士最终把整个国家拖垮。
